REQUEST FOR PROPOSAL (RFP)

ASSERTIVE COMMUNITY TREATMENT TEAM (ACTT)

DECEMBER 19, 2004

                                                                                                                                                                                   Cumberland County area Authority is seeking to contract with an organization/agency with documented experience providing services to adults with Severe and Persistent Mental Illness who may also have a co-occurring diagnosis of Substance Abuse or Developmental Disabilities.  The targeted population has a pattern of frequent use of Crises Services, repeated hospitalizations or incarceration, have failed to engage in conventional services, and have been determined to have complicated needs.

The service is operated through an Assertive Community Treatment Team (ACTT).  This service is provided by an interdisciplinary team that ensures service availability 24 hours a day and is prepared to carry out a range of functions wherever and whenever needed for the consumers served by that team.

Service objectives for this population include: 

  • Preventing or reducing symptoms or behaviors that may result in need for recurrent use of inpatient services or incarceration;  AND
  • Increasing the skills and behaviors that enhance the individual’s ability to remain in the community

The service must comply with all the applicable rules of licensure and operation including but not limited to APSM 30-1: Rules for Mental Health, Developmental Disabilities and Substance Abuse Services, Section .5700-Assertive Community Treatment Service (ACTT).

ACTT is a combination day/night and periodic outpatient service model.  The service is Medicaid billable. The current Medicaid rate for this service is $827.55 per month.

Letters of intent and written questions regarding the RFP should be directed to the person named below no later then January 5, 2005.  A question and answer session for those providers submitting letters of intent by the deadline noted will be held at 109 Bradford Avenue, Fayetteville, NC from 10:00 am – 11:30 am on January 11, 2005. A summary of responses to written questions will be provided to those submitting letters of intent.

A copy of the FRP will be forwarded to all individuals submitting a letter of intent.

The completed RFP must be submitted no later than 5:00 pm, January 18 2005 to the person named below.
